Registration will be May 28, 6:00pm-9:00pm, at the Oakwood Sports Center-Glastonbury, 40 Oakwood Drive, CT 06033.  You must have the following items with you in order to register your team prior to participation:

1.)                  Final Tournament Roster Certified with jersey numbers

2.)                  Current Player Passes

3.)                  Medical/Liability Release Forms –You may use your own club form

4.)                  Permission to Travel Forms (Out of State teams only).

5.)                  Written Permission from guest player’s home club per CJSA rule 3310. (Connecticut

     Players only)

If you are unable to register your team during this period you must notify Matt Cameron prior to May 8th at mattc@oakwoodsoccer.com. On site registration will be available a minimum of 45 minutes prior to your first kick off at the tournament tent at your playing site
